If you are a current member of the Florida National Guard, please download the form below and contact your unit for your Family Assistance Coordinator (FAC)/Family Readiness Coordinator (FRC). The FAC/FRC will let you know the process and review your documents before applying.

WHO IS ELIGIBLE?

Current members of the Florida National Army and Air National Guard

ELIGILITY FOR ASSISTANCE

Foundation accepts applications for Acts of God (accident, injury, illness) or Fault of Guard

Assistance is provided to help with basic living essentials, housing, food, electricity, etc. Bills are paid direct and gift cards are provided for food and gas

Applications will be reviewed on a case by case basis

Applications must be submitted by the service member’s Soldier and Family Readiness Specialist (SFRS). Service members should contact their unit to find their respective SFRS.
